Dayton's Are Driving Innovation in Big Toe Joint Fusion: New Research from the Foot and Ankle Center of Iowa

We are proud to share the latest peer-reviewed research publication from Dr. Paul Dayton and Dr. Mindi Dayton, leaders in foot and ankle surgery and nationally recognized experts in bunion and arthritis treatment. Their recent study, now available here, sheds light on the critical importance of sagittal plane positioning in Triplane First MTP Joint Fusion—a surgical procedure used to treat arthritis and deformity of the big toe joint.


The first MTP (metatarsophalangeal) joint fusion is considered the gold standard for painful arthritis of the big toe joint, whether or not a bunion is present. While this procedure has long been known for its pain-relieving benefits, our surgeons have refined the technique to not only address pain but also optimize function, gait, and long-term satisfaction. This new study explores how the up-and-down (sagittal plane) alignment of the big toe during surgery impacts patient-reported outcomes—including functional use of the foot, walking ability, and overall satisfaction. The data confirm what our team has observed clinically: proper sagittal alignment plays a key role in restoring natural function and improving quality of life post-surgery.


At the Foot and Ankle Center of Iowa, our team has pioneered a triplane approach—correcting alignment across three planes of motion—and developed a reproducible method to drive precise correction of the first metatarsal. A cornerstone of this technique is our dual-plate fixation method, which we have used for over 15 years.
